1. HEALTH assessment. Diagnosis and plan for treatment 2. ASSESSMENT of home environment for factors that MAY contribute to health and safety problems 3. ONGOING medical care AT patient various homes 4. SUPPORT and education to patient and their families SO they CAN make informed healthcare decisions 5. CARE management and oversight. Both within the Patient’s home across settings for example. From home to hospital and rehab facility and back home again 6. COORDINATION of health services with visiting nurses. Medical specialists. Hospital staff. Hospice services and community agencies to improve the continuity of health and medical care and facilitate hospital admissions whenever necessary 7. ASSISTS patients with identifying medical equipment needs. Authorize and oversee durable medical equipment 8. CONDUCTS routine tests such AS blood work and urine analysis 9. MAKE referrals for physical therapy. Speech therapy. Occupational therapy following an acute illness or medical EVENT

HOME and community based services (HCS)
•RESIDENTIAL assistance services. Which are supervised living. Residential support services and FOSTER/COMPANION care. We offer this service in a group home or FOSTER/COMPANION care home. •DAY habilitation. YOU will BE transferred to and from the rehabilitation center where YOU CAN learn NEW skills in a group setting. •supported home living. We assist YOU to live independently or with Family or friends. •respite care. We provide assistance whenever needed to help your Family or caretaker break from caretaking. •specialized therapies.
