Community Blue is a health insurance plan offered by BlueCross BlueShield of Western New York and is located in Buffalo, N.Y. It comprises all commercial health maintenance organization products that are community rated for all groups. Community Blue s HMO plans focus on wellness and prevention while providing affordable health coverage. Additionally, members receive an open-access health plan that does require referrals with participating specialists. It also offers members a point of option service option where members can see a non-participating physician, without a referral, at an additional cost. Community Blue s HMO plans include 100 and 200 Plus series. The 200 Plus series has a large provider network and offers benefits that include choice of co-pays, out of network options, vision coverage and dental care. BlueCross BlueShield of Western New York was established in 1936 and offers several health care services, including personal health concierge, disease and care management, and physician and hospital quality incentive plan.
