Located in West St. Paul, Minn., Eldercare Partners is the unity of five nonprofit organizations and has been serving for more than 25 years. Its caregiver services include basic care and caregiver coaching and counseling. Eldercare Partners offers caregiver training in nursing and therapy. The organization s geriatric care management services include office consultations, assessment and recommendations, service planning and coordination, and ongoing monitoring and coordination. Eldercare Partners s care management services also include family meetings attending, doctor s appointments checking, completing paper working and home maintenance.
