Health Care for the Homeless provides medical-related, educational and advocacy services to reduce the incidence and burden of homelessness in Maryland. offers mental health, social and addiction treatment services. Health Care for the Homeless serves more than 8,500 individuals annually at clinic sites in Baltimore, Frederick and Montgomery counties in Maryland. Its Connect and Adherence Projects promote residential and medical stability for vulnerable individuals living with HIV. The organization administers the Convalescent Care Program, which ensures nursing assessment and recuperative care for individuals with medical conditions. It is a part of a project funded by the Robert Wood Johnson Foundation, Pew Charitable Trust and US Conference of Mayors.
