Heartland Community Health Center offers -primary care medical clinic -a food pantry (by appointment only) -counseling services -financial benevolence -health programs (such as the Diabetes Care & Prevention Program and the Wellness Program) The clinic charges a fee based on a sliding scale. All other services are free.
