Hospice is a fancy word for keeping people comfortable when they have a life-limiting illness. Our caregivers will visit your home and help keep you comfortable during this difficult time.
Comfort care is meant to help you make the most of the last months of life by giving you comfort and relief from pain. The focus is on care, not cure.
How does it Work?
Your doctor and the Heartwood team will work with you and your family to set up a plan of care that meets your needs.
Your primary caregiver may be a family member or other person who cares for you every day. We don?t replace your caregiver but instead are there for support and as a resource twenty four hours each day.
